I think (AC5) Revenge of Rusk also has some (I put AC5 in brackets because my copy says AC3 which was the Kidnapping of Princess Arelina).

M4 Five Coins for a Kingdom has tokens for the 5 coins (cut out from the cover).

Several other modules have (photocopy and) cut out player handouts and maps - X1, X6, X9, CM1, M3, M5 etc. - not sure if you count them.

Orcwars can also be found (rules, board, and chits) in Dragon Magazine #132.

Module O1 The Gem and the Staff (for Expert) was a one-on-one adventure with stand-up miniatures and a booklet of 1"=5' battle maps of each room.

If we include Dungeon Magazine adventures there were poster maps included for Them Apples (#48, set in The Five Shires) and Elexa's Endeavor (#53, set in Karameikos IIRC) and possibly others as well.

EDIT: I just remembered The Fountain of Health (#39, set in Glantri).

And to clairfy: These posters were all battle maps in the style of Zanzer's Dungeon from the black box.

Wow, there was a lot more here than I remembered. That is awesome! :)

Lets leave out regular maps, since that would include pretty much everything, but battle maps like the one from B10 is definitely in! :)
