Too hard to choose just one, as was pointed out in the PHB thread.

TOP 5
- Medusa on p83
- Knight on p98
- Underwater thief p117
- The gate p124
- Wizard laboratory p 215

Best Party Illustration:
- The trio on p 243

Special Mention :
- The Magic items section for its generous, beautiful and full color illustrations.

Mapping :
- The birds eye view of the fortified town on page 115

Best Spoof Award goes to :
- The LOTR composition of the Modrons searching for the adventurers on page 42.

I know that is more than 5... ;)

So I just flipped through the whole book to come up with my top 5 - and it ended up that you already had 4 of them (pgs 83,98, 215, and 243). I also whole-heartedly agree with the special mention of the magic item section. Fantastic!

The one that you didn't mention that made my top 5, was pg 96, the undead (and/or death cleric?). I love the composition of its skin and overall pose.

It is a very good illustration. The way the light reflects on the purple-gray skin gives it a very cryptic ambiance.

The spellcaster on page 283. I like the lighting and the palette.

Page 7's chapter opener. That would be fun to map. For the PCs, at any rate ;)

Page 111's "environments" illustration. I get cold just looking at it. Which, in the Texas summer, is a good thing.

Here's my top 5:
91, beholder
96, lich
98, knight on leaping horse
259, mad king
311, "The" DMG dungeon map

Honestly, none of the art really stands out for me, but at least I don't hate any of them outright for not living up to my fantasy ideals.

My top 5, after a very quick flip through the book:
69, tarrasque (although I hate its stat block in the MM!)
73, grinning gargoyle face cave (because anyone with ranks in game history should be nervous...)
117, underwater thief
186, portable hole (because that item's much harder to render than you'd think!)
202, staff of healing (because the idea of the cleric coming anywhere near their wounded friends with such a creepy stick amuses me greatly)
