Is there an [online] index I can peruse to get an idea of module levels, setting intended to be run in, etc?

This site has a document detailing all the 3.X adventures (including, but not limited to Dungeon magazine) with the info you're looking for. I'm uncertain about other editions of D&D, though.

Interesting question!
This one seems only to issue 81, i.e. the D&D and AD&D era:

These two cover up to issue 150, i.e. the whole print run:

The file linked by enderxenocide0 seems to cover the 3ed era only.

It seems there isn't any index for the web issues 151-221 (4ed), I cannot even find them online anymore (at least not officially), but I have them and I can say there is much generic, some Nentir Vale, some Eberron, some Forgotten Realms, some conversion of old adventures. From a certain point there are also regular columns on the Forgotten Realms, Eberron and Dark Sun, and Dark Sun adventures. At least one set in Ravenloft in issue 207.
I think Greyhawk is never explicitly mentioned, but there are several adventures updating Greyhawk locations to 4ed.
