Uhm, while the Zothique cycle is related to the Mythos, there are few direct references.

If you're willing to include unpublished material I have a version based on Basic D&D mixed with bits of Swords & Wizardry. We've played it in the classic 1920s, dark ages, vietnam, western, victorian, WWII, and modern. We enjoy the familiarity of old D&D and that makes it easily compatible with Realms of Crawling Chaos, Silent Legions, etc.

Well, a reference to unpublished material is not that useful, since no one can actually look at it... nice to see you've been able to run a Lovecraftian game with Basic D&D, but at the present time other people willing to do the same need to homebrew it or use Silent Legions etc..

Wildfire, the company that did CthulhuTech and Chthonian Stars, has also done The Void, which (based on the blurb) appears to be an update of Chthonian Stars to use a new d6-based system.

My Dodécaèdre includes Mythos deities along with Greco-Roman, Norse, Semitic and some others. Mythos cults are generally forbidden, at least on the 2d face of the world, but act under other deities disguises.

OneDice WWI includes an Occultish Horror "skin" (basically, a layer of optional rules to reflect the genre) that heavily features Lovecraftian monsters and themes. This book may have the shortest stat blocks I've ever seen for the Sanity-blasting horrors of the Mythos! ;)
